Interactive product label (webpage, which prints given values to a pre-defined visual template)

We are looking for a solution for producing an interactive product label. The visualization of this label already exists in a vector format. We are looking for a person to create a website, which would allow a user to type in 4 values, and the page would modify the label accordingly.
